Help with Applications

This page collects information on the location you are completing the application from and whether you require an authorized representative to assist in completing this application/LaCAP Enrollment.

Some items have an asterisk (*) next to them, indicating they are mandatory. You must fill in these items before you can continue to the next page.

 

Application Location

 

If the location where you are completing the online application/LaCAP Enrollment is not listed in the drop-down menu, select Other. A text box appears to type a new specific location.

 

Using CAFÉ Customer Portal

 

Click in the circle next to the option that best describes your situation for completing the online application.

Selecting the Community Partner or DCFS employee assisting a customer in completing the online application will cause the text boxes to enter additional information to appear.

If you are completing the online application from a community partner site, enter the Community Partner Site ID in the text box. A representative at the Community Partner site can provide the

 

Community Partner Site ID

 

If you are a DCFS employee and assisting a customer in completing an online application, choose option 4 and enter your first and last name in the DCFS Employee text boxes.

 

DCFS Employee

 

If “I am a DCFS employee assisting a customer” is selected, you must enter First and Last Name of the employee in this section. The Middle Name is optional.

 

Community Partners

 

If “I am a community partner assisting a client” is selected, you must enter the Partner ID of the community partner who is assisting.

 

Authorized Representative

 

If you choose an Authorized Representative, you will have the opportunity to enter more information about this person on the next page. An Authorized Representative is an individual who has been authorized by an applicant to represent them in the application process.

 

Click the Print button to print this page for your reference.

 

Click the Next button to save and continue with the online application/LaCAP Enrollment.

 

Click the Save & Exit button to exit the online application/LaCAP Enrollment and save your progress on your application/LaCAP Enrollment. The application/LaCAP Enrollment is saved as Incomplete.
