On this screen, enter information about the people in your household. If No was selected for Do you have additional household members? on the Other Information screen, this screen does not display, and you will navigate directly to the Household Summary screen.

Enter the following information for each person in your household:
First Name: This field is mandatory
Middle Initial
Last Name: This field is mandatory
Suffix
Personal Details
Enter the personal details for each person in your household:
Date of Birth: This field is mandatory
Social Security Number
Louisiana Driver’s License or State Issued ID
Gender: This field is mandatory
Race: Check all applicable options
Do you have additional household members?: This field is mandatory. Select Yes or No from the drop-down list. Select Yes if there are additional persons that purchase and prepare food together and currently live with you.
Click Previous to return to the Other Information screen.
Click Save & Exit to save your place in the registration and return to the home screen.
Click Next to continue.
