On this screen, enter personal information for the person you consider head of household for your family.

Name: Enter first and last name of the head of household

Personal Details: In this section provide the head of household’s:

• Date of Birth: This field is mandatory
• Social Security Number
• Louisiana Driver’s License or State Issued ID
• Gender: This field is mandatory
• Race

Parish of Origin (Home Parish): This field is mandatory. Select Parish of Origin (Home Parish) from the drop-down list.

Click Previous to return to the What is DSNAP? screen.

Click Save & Exit to save your place in the registration and return to the home screen.

Click Next to continue.
