Link Accounts to Cases

This page is the first step towards linking your case information to your CAFÉ account. In order to link your existing case(s) to your account or to begin to submit an application for continued assistance, enter the following information:

 

 

Once you have entered valid data, click the Next button to submit this information. If you receive an error indicating that no cases were found, check for accuracy of entered information and try again. If the information entered is correct and you still receive an error, you may not have existing cases to link to your account.

 

Click the Next button to save and proceed to the next page in the process.
