My Simplified Reports

The My Simplified Reports page allows you to submit a Simplified Report, return to an incomplete Simplified Report that has not been submitted, view cases linked to your account or view recently submitted Simplified Reports within the past three years.

 

If you are currently receiving benefits or services and wish to submit a Simplified Report, click the “Submit Now” button. If it isn’t time for you to submit a Simplified Report, an error message will display telling you that you cannot proceed at this time. If your account is not linked to any case, clicking the “Submit Now” button leads you to the Link Account to Cases page.

 

If you have any programs that are currently linked to your account, they will display in the Program(s) Linked to Your Account section. If you would like to link any programs to your account, click “Add Other Case(s) to Your Account” to access the Link Account to Cases page.

 

If you previously started a Simplified Report and had to exit and save, click “Continue” next to the appropriate report under Incomplete Simplified Reports. This will take you back to the page you last saved on in the application.

 

The Submitted Simplified Reports section displays a summary of the Simplified Reports and programs you have submitted in the past three years.

 

Click the View Full Summary link to view and/or print a full summary of the Simplified Report.

 

Click the Check My Cases link to go to the Check My Cases Home Page.

 

Click the Back to MyAccount button to return to the MyAccount Homepage.
