Dependent Care Details (Change or End)

This page displays if you selected “Change” or “No longer has this bill” on the Other Expense Information page. If this is incorrect, return to the Other Expense Information page to correct the bill information.

 

Some items have an asterisk (*) next to them, indicating they are mandatory. You must fill in these items before you can continue to the next page.

 

If you selected that you no longer have this bill, enter the date the bill ended. If you selected that the bill has changed, enter the date the bill changed.

 

Enter the information regarding your bill that ended or changed.

 

Click the Print button to print this page for your reference.

 

Click the Next button to save and continue with the online Redetermination/Change Report.

 

Click the Previous button to view the previous page.

 

Click the Save & Exit button to exit the online Redetermination/Change Report and save your progress on your Redetermination/Change Report. The Redetermination/Change Report is saved as Incomplete.
