This page collects information on how new members of the household are related to one another.
Some items have an asterisk (*) next to them, indicating they are mandatory. You must fill in these items before you can continue to the next page.
Select the relationship type from the relationship drop-down menu for each household member. If the relationship of the household members is not listed in the relationship drop-down menu, select Related in another way.
This page will appear multiple times until all relationships in the household are established. Relationships are collected based on how each individual is related to the applicant.
If you indicated that someone was pregnant, you must enter the due date of the baby.
Click the Print button to print this page for your reference.
Click the Next button to save and continue with the online Redetermination/Change Report/Simplified Report.
Click the Previous button to view the previous page.
Click the Save & Exit button to exit the online Redetermination/Change Report/Simplified Report and save your progress on your Redetermination/Change Report/Simplified Report. The Redetermination/Change Report/Simplified Report is saved as Incomplete.
